Providing Glasses in the US How to Apply for Assistance

IMPORTANT: New Eyes for the Needy is unable to accept individual applications for eyeglass vouchers due to a decline in funding and an increase in requests for assistance.

If you have a social worker, case worker or agency that you work with, they may be able to help you submit an Agency Application.

Agency applications will be reviewed. If approved, the agency will receive a voucher on behalf of the client.

Applicants from Kentucky

Clients in Kentucky receive their vouchers through Kentucky Homeplace, a program that links medically underserved rural residents with free or low-cost health services.

If you live in Kentucky, you must request an application from Kentucky Homeplace by:
